Submit Passports or Government IDs

To comply with the TSA’s Secure Flight program, a government issued ID must be provided and must match the government-issued photo identification that will be used for travel. Momentum Vacations is required to collect travelers’ full name, gender and date of birth as these elements appear on the government issued ID that you present during travel before we can book any reservation.

Submit Passports/Government ID

All names should be submitted as they appear on your government issued ID. Include middle name or initial, where applicable. Passport photos should be clear and legible. Acceptable image formats are JPG, PNG, GIF and PDF. If you need to submit more than five (5) passports, please submit multiple forms.
